Notice to Patients

• PHSC serves all patients regardless of ability to pay.
• PHSC offers discounts for health care services for those that qualify. Applications for discounted fees are available and will be determined by family size and income.
• PHSC does not discriminate against patients on the basis of race, color, cultural heritage, national origin, religion, age, gender, sexual orientation, marital status, physical or mental disability, political affiliation, source of income, source of payment, veteran status, or any other status protected under local, state, or federal law.

Availability of Language Access Services (LAS)

PHSC provides free language services to peoplel with disabilities and to people whose primary language is not English in order to have effective communication between staff and patients.  These services include:

• Qualified interpreters (including American Sign Language)
• Information writte in other languages
• Information in other formats (audio & accessible electronic formats)
